Most reports and forms are completed within days of your evaluation and records review. Because agency deadlines are tied to each test administration, we recommend starting several weeks ahead of your filing deadline.
Evaluations are conducted via secure telehealth for residents of Louisiana and California.
These evaluations are self-pay. We can provide a superbill you may submit for possible out-of-network reimbursement, though accommodation evaluations are frequently not considered covered services. You’ll receive a Good Faith Estimate of expected charges before any evaluation begins.
A $500 retainer is collected when you schedule, to reserve your appointment, and is applied in full toward your evaluation fee. The remaining balance is processed on the day of your evaluation. Evaluations are self-pay, and you’ll receive a Good Faith Estimate of expected charges before anything is scheduled.
