Two transparent options, each including all required forms and letters. A Good Faith Estimate is provided before any evaluation begins.
A $500 retainer is collected at the time of scheduling to reserve your appointment and is applied in full toward your evaluation fee. The remaining balance is processed on the day of your evaluation. The retainer helps ensure appointment times are held for clients ready to move forward.
These evaluations are self-pay and are not billed to insurance. In accordance with the federal No Surprises Act, every self-pay client receives a Good Faith Estimate of expected charges before any evaluation begins. A superbill can be provided for possible out-of-network reimbursement, though accommodation evaluations are frequently not considered covered services.
Your fee covers a rigorous, physician-led evaluation and the complete documentation package — report, forms, and letters — built to your exam’s standard. It does not buy an outcome: accommodation decisions rest solely with the testing agency, and no ethical provider can guarantee approval.
If your situation calls for a formal cognitive or achievement battery (for a suspected learning disorder, for example), we’ll tell you during the complimentary eligibility review and refer you to the right evaluator — at no charge.
Tell us your exam, your timeline, and a little about your situation — we’ll confirm whether this is the right path before you commit.
Check your eligibilityImportant: This page is informational and does not constitute medical advice or establish a provider–patient relationship. Completion of an evaluation does not guarantee approval of any accommodation; all decisions are made solely by the relevant testing agency or institution, and past approval outcomes do not guarantee future results. Evaluations are available only to residents of states where the provider is licensed (Louisiana and California). A Good Faith Estimate of expected charges is provided to all self-pay clients in accordance with the No Surprises Act.
