Book your evaluation

Choose an available time below. After your complimentary eligibility review, this is where you’ll reserve your evaluation visit.

Before you book: a $500 retainer is collected at the time of scheduling to reserve your appointment and is applied in full toward your evaluation fee. Evaluations are self-pay; you’ll receive a Good Faith Estimate of expected charges before your visit, with the remaining balance due ahead of the appointment. A superbill is available on request for possible out-of-network reimbursement. Visits are by secure telehealth for Louisiana & California residents.
